2021.10.19 16:55 Z3ROMAN Clinical Trials - My two cents

My daughter was diagnosed a couple years back. Coming from a family with T1D history it was still a gut punch but one that didn't take days / week to process. I knew what it meant right away. So the first thing I did after having a long honest talk with my daughter about how her life has changed forever was to go on the offensive. I started searching all clinical trials I could find and aggressively contacting them to see what they were doing and if we could participate.
This was two fold. First, if you can get into one of these your chances of getting connected to a really good endocrinologist that actually is up to speed and has expert level knowledge of T1D (don't get me started on how terrible some can be) are really high. We have been surrounded by some of the best thanks to her clinical experience. Second, you get a chance to get into something that could either directly help your T1D or be a part of helping the T1D community in the future. What I found shocking in this process though was how difficult it was to get info or assistance from her original endocrinologist. You are on your own - understand that and act accordingly. You have to do the homework, make the calls and ask the questions. If you get help from your doc consider yourself lucky.
Through my searches I found Provention Bio and was (just barely) able to get my daughter into a new study called "Protect". This company is aggressively attacking T1D from the beginning, not the end. The idea is that they will eventually have a vaccine against the specific trigger event (they are targeting a viral infection) that causes the dominos to start to fall that cause T1D. It's fascinating and they are also doing this with Lupus and Celiac - they are trying to prevent auto-immune disease from ever beginning.

My daughter successfully completed the Protect study earlier this year. It was a double blind study that gave participants different doses of Teplizumab or gave them placebo. It was administered only to newly diagnosed (within two weeks of diagnosis) in two rounds of daily infusions over the course of a week - 6 months apart. The idea is to dramatically extend the "honeymoon phase" of the newly diagnosed by calming down the body's auto-immune system. Because of the side effects she experienced during the infusions we know that my daughter got the actual drug (not placebo) and it has worked. She remains at her same 1:23 ratio she has been since her diagnosis (after a couple of weeks of fine tuning of course). This study was in Atlanta so we had to travel back and forth from DFW but the benefit was the Endo in charge was more than happy to be her default primary care and was the opposite of her endo we had at home. He made sure she had access to all the newest tools, newest insulin, most current knowledge. It was a blessing that would have been worth it even if she got the placebo. Unfortunately the study ended and having your primary Endo in a city you don't live in doesn't make sense. So we came back to our local endo and to just being another file with a passing interest in making sure you are living somewhat healthy and a vanilla mentality of just accepting you have T1D and always will.. But thankfully that didn't last long.
She has recently signed up for an extended monitoring study over the next 5 years that will monitor how long the effects of the Teplizumab infusions last. The good news is that it is here in the DFW area close to where we live and the study doctor happily took her on as his own patient - again being part of a study has side benefits beyond the actual drug / device in the study. Her new doctor is the head of pediatric endocrinology for Children's Health and professor of pediatric endocrinology at UT Southwest... How do you get better than that? We could have never made that connection outside of the clinical trial world.
All that she is doing isn't a cure for her by any means, my daughter is still a T1D. But what little insulin production function her pancreas still had in it has been protected from the body finishing it off. How long? Who knows. The original group before this group had one infusion and it helped up to 5 years. Their hope is that 2 infusions will do at least that much, should be longer, or maybe the body just stops its desire to finish its beta cell genocide all together? But it set's up the next test for the next generation of T1Ds. Through Trial Net they are actively seeking blood samples of those at high risk for T1D so that they can identify (through antibodies) those that will get T1D but aren't clinical yet and get them on this drug or another promising drug to see if they can actively delay or prevent onset of T1D. Imagine if you could go back in time and have regular blood work done that identified your body was ramping up to kill your beta cells and the doctor could simply write a script for a drug that would block it from ever happening? We are close.
I share all of this information not just because it is fascinating and I truly believe we are closer to a cure than we have ever been. I share it also to ask you to be part of the solution and to possibly open doors you might not have known existed. If you have T1D in your family PLEASE get the other family members tested. They want to test your blood for anti-bodies to build the database for studies and possibly for candidates for the "prevention" or another study trying to do the same thing - find a cure. If you already have T1D search the clinicals that are happening. CGM implants, advanced pumps, new forms of insulin - they need people to test them.

T1D has effected too many people in my life. I want to hear "we have found the cure" in my lifetime so that no parent has to go through that gut punch again. While this study that my daughter participated in can't fix her condition (she is still going to be T1D) - it helps pave the way for the next generation. But, as soon as she is old enough and the process is refined enough we will hopefully someday use my daughter's stored stem cells to rebuild her own beta cells and we will owe that opportunity to someone older than her that signed up for a clinical study that probably didn't do much for them but it paved the way for those that came after them....

My (23m) gf (22f) and I have been dating for 1 year. Prior to dating her I had my best friend (23f) who I've known since we were 15. Nothing sexual or romantic ever happened between us, all things platonic. We were just very touchy with each other and since we were both single we always hanged out with each other, our friends assumed we'd get together sooner or later, our parents thought the same, my best friend's parents wanted me as their daughter's boyfriend and my mom adored my bff. But we still remained friends.
So last year I got with my current gf. I didn't change anything with my bff in terms of how often we hang out and how we act around each other until few months in my gf made complaints about how she feels uncomfortable when I do x stuff with my girl bsf and how I'm disrespecting the relationship because it looks like I'm dating my bsf. This is absolutely not true and I don't believe it ever looked like that but I respected my gfs concerns and tried to be less touchy and hang out less with my bsf and tries to focus more of my attention to my gf.
Apparently I didn't give any attention to her either. She claims I only said I'd change and I never did. She kept saying she felt like she's third wheeling when she's around us. I never felt like that and I told her to cut that shit off and accept that she's my friend and I won't change the way I act around her. She seemed to tone it down for a while.
Last weekend it was my bsfs bday party and we went to her house and her parents were there. Her father greeted jokingly said "Where's my future son in law" and my bsf laughed while my gf looked like she was about to cry. She wouldn't speak all night and I asked her what's wrong and she said she feels horrible and shouldnt have come to the party. I said we'll talk about it once we leave.
We left, went to my car and she burst into tears saying how she feels like I'm using her until I can get with my bsf and how she feels disrespected by the jokes my bsfs dad made and how my bsf laughed and never respects boundaries, she blamed me for not enforcing boundaries and said that unless I set some things straight with my bsf she'll break up with me.
I'm tired of her always complaining about it and not just accepting that this is my friend, who I've known longer, I love my gf as my gf and my friend as my friend but she shouldn't expect me to change around my bsf so she should feel comfortable. She makes me feel controlled and I want to have a talk with her about her behavior but idk where to start and the right words to say. How do I deal with her?
Tldr ; gf is jealous of my relationship with my girl bsf and said she'll dump me unless I set boundaries

I'm trying to figure out a mobile workflow that can allow me to edit photos when I'm not at home. Lightroom Mobile on the iPad is great, but it doesn't look like any mobile Adobe software has panorama stitching functionality. What do you use?
99% of the time I'm only bringing a 50mm lens, so for landscapes it's critical that I have a panorama stitching functionality. I'd rather not do it in-camera either.

"Halloween Kills" takes its title literally. From the very first scene and to the very last one, death and mayhem surrounds this film. And while the remorseless kill count and bloody gore is probably at an all-time high, those seeking a more meaty story will find "Halloween Kills" a middling and even disappointing middleground.
They thought they killed him but the unstoppable Michael Myers once again survives a seemingly fiery fate. While Michael continues his onslaught through the town of Haddonfield, Laurie Strode and her family are taken to the hospital as Laurie recuperates from her injuries. When it is clear to the townsfolk that Michael Myers is really back in town, they decide to take matters in their own hands. Can Michael Myers be finally stopped through sheer grit and grind or will the kill count just grow exponentially from here?
After the excellent "Halloween", "Halloween Kills" felt like a step back for the series or at least it was obvious that it was the middle film in a planned trilogy. Aside from an excellent and mesmerizingly accurate callback to 1978 (this was easily our favorite portion in the film), there is little that was added to the story of Michael Myers. This sequel focuses solely on what Michael Myers is known for and is it relentless, unstoppable, remorseless murders. If that's what you're looking for then this sequel won't disappoint. The kills are not only endless but it they were creative, fresh, and disturbingly believable. Likewise, the visuals and soundtrack was extremely well done and added to the unique atmosphere that this franchise is known for. Unfortunately, most of these deaths do occur because of sheer stupidity. This was a film that had us rolling our eyes and screaming in frustration as these Haddonfield folks just lure death right into their doorsteps. Aside from the kill count, "Halloween Kills" failed to slay it when it came to its story. This film just didn't go anywhere narratively-speaking. It had some interesting takes on how people became monsters themselves as they took justice into their own hands but even this subplot was half-baked in our opinion. Like Laurie Strode who's injured and unable to do anyting significant, "Halloween Kills" felt pretty much the same. It brought just the basics to make this worthwhile and enjoyable but not much else to make it standout and be memorable.
